This item is "loot" from the trading card game. Whenever you buy a pack of World of Warcraft cards from a store, there is a chance to get a "loot card" inside. It's a low chance to get a loot card in the first place, let alone a good loot card such as Tyrael's Hilt. On loot cards there is a scratch off part and underneath is a code. You have to go to the WoW trading card game site to enter that code, then they will give you a NEW code. You'll then enter that code at the vendor in Booty Bay and you will receive your loot. You will only receive the loot on the character that you entered the code on, and you can't trade the loot with anyone.

Tyrael's Hilt was not a WOW TCG Card from a deck. It was given out at the Paris Blizzcon in 2008. The only place you can find it now is either on ebay or a reputable WOW TCG Selling site, but be careful of scammers.
