Kawaii does mean cute, but Kawai and Kawaii are both relatively common family names in Japan that use Kanji that have nothing to do with cute. So it might just refer to Ms. Kawai or Mr. Kawaii and not "Mr/s. Cute."
Mm, kawaii is "i adjective" so it's "cute" you don't add "san" to adjective, only to name. But, I think this person wanted to call someone, "Mr/Miss cute."
Could be arogant.

Kawaii is the name of a main island of Hawaii...and when you add -san to the end of a name..it shows respect..

"Kawaii" is the person's surname. It doesn't mean "cute" etc. as in かわいい/可愛い 。It could be 河井伊、Or 川伊井 or other different kanjis.
"kawaii" normally means 'cute', 'pretty', 'precious', 'lovable', 'adorable', or 'innocent'.
"-san" is the most common honorific and is a title of respect.
So it could mean “Mr. Cute” or “Ms. Cute”.
