What happens if you have missed jury duty (California)? Has this happened to you?
I missed my jury duty and received a notice in the mail stating: "Your failure to appear in court is punishable by contempt of court and/or fine, and failure to respond to this notice may result in the court ordering monetary sanctions." This sounds very serious. I acknowledge that I should have attended, but now I’m unsure about what to do next. Have you or anyone you know experienced a similar situation? Will I get into trouble if I reach out to the court myself? Alternatively, could ignoring this issue lead to more severe consequences? I'm looking for serious answers only, please.

From what I have read California is the most strict about this. Some States and County's will not bother You. It is best to not take a chance.The Law states In Illinois that You may be fined no more than $100 and/or no more than 3 days in jail. If You call and say You forgot They will just reschedule You. If You do nothing They will send a Sheriff out with an arrest warrant to find out why.
I skipped jury duty in California once. About two years later I got a notice to appear for jury that included a threat of a fine and/or jail if I didn't appear. I appeared.
It is illegal to skip out on jury duty. Punishment is up to the judge
So, it is serious, so it's best that you do whatever that letter says. I would call the clerk and choose another when you will show up. Tell them why you missed jury duty, apologize and MAKE SURE you show up on your new date.
If you don't do anything after that letter, it's very possible they put out a bench warrant for you, or even put you in jail, and I'm not kidding.
But I do think that if you tell them you'll show up on another date, it should be OK.

you could go to jail for contempt of court, I'd imagne. Not to mention the embarassment when the police come to get you to take you to court.
